We are pleased to offer Nationwide Nutrition Counseling with Jim Golick, CC, LDN. One-hour counseling sessions can be conducted by telephone or in-store at any one of the 11 Fruitful Yield Health Food stores in the Chicagoland area.
How do I schedule an appointment with Jim Golick?
There are two convenient ways to schedule an appointment for a telephone consultation OR a Chicago area in-store consultation:

Whether you call Fruitful Yield, or send an e-mail to schedule your appointment, please note:
- your preference for appointment dates
- you preference for appointment time (include time zone you are in)
- phone number (two numbers preferred) where you can be reached for a call back
After receiving your message, Jim Golick will normally call within 1 business day to schedule your appointment. This timeframe may vary depending on call volume.

How much does nutrition counseling cost?
- If you are a Chicagoland area resident and would like to schedule an appointment for in-person consultation at one of the 11 Fruitful Yield or Here's Health vitamin stores, the discount rate is $20/session.
- Standard rates to see a licensed nutritionist in private practice start around $75/hour. As part of the NOW Foods mission, to provide value in products and services to empower people to lead healthier lives, we offer telephone nutrition counseling for only $30/session.
How many nutrition counseling sessions will I need?
- Most people will desire 2 to 3 appointments over a period of 3 - 4 months, but this can be mutually decided upon as needed.
What do I do after I schedule my appointment? After your appointment has been scheduled you will be required to pay for your nutrition counseling session prior to your first session. You will also be required to complete a 4-page questionnaire about your current health conditions, diet, exercise, medications and supplements prior to your first session. How do I pay for my nutrition counseling sessions? For telephone or in-person consultations, you will be required to pay by credit card prior to each of your nutrition counseling sessions.
- To pay online, please use The Fruitful Yield secure, online payment system to process your credit card.
- To pay by phone, please call 1-800-469-5552, Monday thru Friday, 9:00 a.m. - 5:00 p.m. CST. Please have your credit card information ready. The person you talk to will take your credit card information, but will NOT schedule your appointment. Jim will schedule all appointments.

Where do I get the questionnaire that I am required to fill out prior to my first session?
- Ask Jim to e-mail you the WORD document as an attachment at the time you schedule your appointment. You can fill out the questionnaire electronically and then e-mail it back to Jim.
Where do I send the completed questionnaire?
- Request that the questionnaire be emailed to you as a WORD doc when you schedule your appointment and then email the completed form to Jim Golick at vitaman5455@comcast.net; OR
- If you have an in-person consultation, please bring the completed forms with you on the day of your first session.
*NOTE: Please include copies of blood work or other relevant lab tests you have had within the past 6 - 12 months. What happens at the end of my first counseling session? At the end of the first nutirion counseling session, you will be given various options to pursue. Among these will be a list of recommended foods and dietary supplements to purchase. By special arrangement with the Fruitful Yield Mail Order division, all of Jim's clients will receive FREE SHIPPING (within the continental U.S.) on all products purchased (a $5.25value) through The Fruitful Yield Mail Order division. Free shippping will apply to all purchases made within 30 days of your nutrition counseling session. Just mention Dept. 10 when you place your order.
